Targeted care packages are an opportunity to be more innovative, flexible and client–centrered to the living arrangements and care needs of young people transitioning from residential care to alternate living arrangements and to prevent the entry of a young person to residential care. Funded by the Department of Health & Human Services the package is individualised to the young person.
