Better Futures is a program for young people post Department of Health and Human Services (DHHS) care. Program is to support young people aged 16-21 years so they are able to live independently of DHHS. This can be a time where additional support is often needed, for full independence to be achieved.
The young person is allocated a case manager, who assists them in reaching their independent living goals.
Referrals are received from DHHS while the young person is in care. If the young person has left care, and aged under 21 years, they are able to self refer.
