Leaving Care (LC)
A program for young people post Department of Health and Human Services (DHHS) care. Upon a young person turning 18, they are able to live independently of DHHS. This can be a time where additional support is often needed, for full independence to be achieved.
The young person is allocated a case manager, who assists them in reaching their independent living goals.
Referrals into this program can be be made as a self referral by a young person or sent from an agency/organisation.
